Student Folder Cover Letter A letter introducing teachers serving students with hearing loss to the documents regarding deaf/hard-of-hearing services the student will receive.  
What is the IEP? A letter introducing the legal requirements of the Individual Education Plan (IEP) and the relationship to a student's educational program.  
Tips for a Successful Learning Environment for Students with Hearing Loss A form listing tips to improve student potential for success in a general education setting.  
Introduction to Providing Consultation - Elementary Introductory information regarding a deaf or hard of hearing individual to be given to any and all teachers of an elementary student served on consultation, at the beginning of the school year.  
Introduction to Providing Consultation- Secondary Introductory information regarding a deaf or hard of hearing individual to be given to any and all teachers of a secondary student who is served on consultation.  
Introduction to Providing Consultation: Unilateral Loss - Elementary Introductory information regarding a student with a unilateral loss who is served on a consultation basis at the elementary level.  
Introduction to Providing Consultation: Unilateral Loss - Secondary Introductory information regarding a student with a unilateral loss who is served on a consultation basis at the secondary level for any and all teachers.  
Consultation Staff Follow-up A form to communicate with the teacher(s) of a student on consultation after a conference. Used especially with secondary teachers.  
Example Consultation Survey/Report A form to provide teachers serving a student with hearing loss to obtain information about their classroom activity.
